Drug Policy and Consent Form
We must receive this form before your job application can be considered. By submitting this form, you are agreeing to provide Custom Aircraft Cabinets, Inc. with the names of all companies for whom you have worked in the past two years. CAC requires all potential safety sensitive employees to take a pre-cmployment drug test following DOT/FAA guidelines. Before taking the pre-employment drug test, you are required to pay a $55 deposit to our office manager. If you should be offered employment, upon notice of a verified negative result and satisfactory results concerning DOT/FAA drug and alcohol documents from previous DOT/FAA regulated employers, you will be contacted regarding your start date and the refund of your deposit. If you receive a positive result on your pre-employment drug test or have any violations to DOT/FAA drug and alcohol policies not listed, you forfeit the refund of your deposit. As an FAA Repair Station, all of our employees are required to participate in random drug and alcohol testing as well as, pre-employment drug testing. We are required by the DOT/FAA to check with any former employers within the past 2 years concerning DOT/FAA regulated drug and alcohol tests results. We are also required to ask all potential employees if they have had a positive or refusal to submit to drug or alcohol test result in the past 2 years when applying with any other company. If you have had a positive test or a refusal to submit to a test, we must have a release from a certified DOT/FAA Substance Abuse Provider before you can be considered for employment. Drugs tested for: Amphetamine, Cocaine, Opiates, Phencyclidine (PCP), Cannabinoid.
This form does not qualify as an application and will not be used in place of the application. Both are required.
